I recently completed a removable upper on 6 implants. The case was designed using NobelGuide to position and place the implants and then the bar was designed with 3 locators in NobelProcera software. This has been the easiest and most accurate case I have done. The absence of casting and joining components, and being able to alter the bar design in real time, made me realise that nothing can compare to this way of doing a case like this. Guarantees on Implants
Most implant companies will offer a guarantee on the implant if you only use their components to finish the case.The question we must ask " is this relevant in today's dentistry"
If an implant is going to fail then how does the abutment contribute to this? An interesting question with very little or no research to follow up with. One reason may well be that the abutment is structurally not compatible with the fitting surface of the implant. This can only occur if the fitting surface of the abutment is changed in any way. To logical thinking by the very nature of investing and casting this may occur. However no matter which cad cam system is employed this is taken out of the equation as files are computer generated.
Then we must address who and how can we trust be sure that the integrity of tolerances of differing implant systems will be part of a finished abutment. One would be confident with a company that is world wide producing abutments in many countries that this is not a problem. To this end I see NobelProcera as a perfect match to allow me to have confidence that I am the "designer" .So where does this leave us with the guarantee. I believe that all reputed implant manufacturers will, as many do now, construct abutments for many platforms and systems giving us the confidence to design in the lab and have produced to exacting tolerances.

Custom Astra Abutment
This Astra Titanium Abutment was designed and created using NobelProcera technology. Please note that the gingival contours resemble the contour of natural gingival tissue, without casting on a gold abutment.

How Crazy is this!!!!
Being able to create individual custom abutments with NobelProcera for Astra,Straumann and Nobel fixtures in titanium for at least 25% less than buying a stock gold abutment then casting onto it and modifying the design to now be an individual abutment is crazy BUT true. The end result for the patient is a more biocompatible abutment created via cad cam to the tolerances of a normal manufactured gold abutment, without the technical difficulties of casting and adjusting on a gold cylinder. To understand that I now have the design capabilities and production criteria as if I was in the design department of a major company is truly amazing.

It's now time for change
I have always been frustrated that many of the popular implant systems do not allow flexibility of abutment design unless you buy a gold non oxidising abutment and then cast onto it. This technique works fine but is time consuming, costly and you still have the end result with gold sub gingivally although acceptable it is not the best material for this situation. Stock machined abutments are limited in design and do not follow gingival contours, as most have a flat horizontal contour.
I am sooooooo happy that now I can create an abutment to suit any contour and have this produced in titanium for the most popular systems in Australia. This saves time , thus money and is clinically more acceptable than gold sub gingivally.
NobelProcera now allows me to create via cad cam technology abutments for :
Astra - all platforms, Straumann - All platforms and of course Nobel- All platforms. Soon others will be introduced.
How exciting is this to be able to create a totally contoured abutment for any case and for most platforms in biocompatible titanium and at a cost effective price. ie cheaper than a stock modified abutment. i think a major step forward in technology.

Abutments continued
So we get the case in do we go to the catalogue or look for another way. Cad cam will allow complete flexibility of design within the lab so both the dentist and the technician can be involved in the design. NobelProcera is the only system that will give you this flexibility. We can scan the implant position in the lab and then via screen shots or direct contact allow the abutment to be designed to the individual case by interaction between both dentist and technician. Using conventional techniques abutments are customised by casting gold onto a preformed cylinder.NobelProcera cad cam, this may I from now on call it a system will allow design to be completed in the laboratory and then the abutment milled at the controlled facility in either Japan or Sweden ( description later) this allows us to be confident that whichever design we create can be reproduced via 5 axis milling machines( again more later ) So is the world changing for the better yes with a capitol Y. I do not work for Nobelbiocare but I run a commercial laboratory that through this blog will look at the benifits of Cad Cam for both us and the clients.

Custom Abutments
I don't think it is common knowledge that a custom abutment can be produced in titanium for a comparable price to a machined off the shelf abutment that needs to be modified to suit the case requirements.
A NobelProcera abutment can be created via cad cam and then produced in my region for a more competitive price than an off the shelf abutment.
To have titanium sub gingivally and then a coping which stays within the parameters of standard crown and bridge protocols is to me the most ideal situation. To many times we try to create restorations that exceed the technical parameters of the materials we have at hand
Thermal expansion of alloys and it's reaction to the bonded porcelain are always a concern
Do we need theses problems!!!!

Hi Bloggers,
I am a new person to blogging thanks to Nick I am now in your world. The idea of this site( if you call it that) is to expand the knowledge and applications of NobelProcera. So you may ask what's this? it is Dental Laboratory technology that will change the way we look at the lab industry and dental restorations as a whole. This blog will cover both clinical and technical knowledge
To let you know I run a commercial dental laboratory in Melbourne, Australia and have been involved in the Procera technology since it's introduction to Australasia yes that does include NZ!!!
